At Pathways Forward, our mission is to plan with and support adults and children with mental disabilities, and their families when appropriate, to live full and inclusive lives within their communities. This will be achieved by providing support to access resources, both within the community and within the public systems for Mental Health, Intellectual and Developmental Disability programs and Early Intervention services. 

Position Overview:
We are seeking a compassionate and proactive Service Access Coordinator to oversee intake, eligibility verification, registration, and waiver administrative functions for children and adults with ID/ASD. This role is vital in ensuring that individuals and families receive the support they need in a timely and person-centered manner.

Key Responsibilities:

  • Provide outreach to the community, raising awareness of our services and engaging with individuals, families, and local entities.
  • Respond promptly to inquiries and referrals through various channels.
  • Conduct initial registration and eligibility verification for individuals requesting services.
  • Assist individuals and families with applications for medical assistance and related benefits.
  • Maintain accurate records in the Home & Community Services Information System (HCSIS).
  • Facilitate communication regarding rights and choices available to individuals and families.
  • Collaborate with various stakeholders to ensure quality service delivery and compliance with regulatory standards.
  • Participate in quality assurance activities and support incident management processes.

Qualifications:

  • Minimum Education: Bachelor’s degree in a human services field (e.g., sociology, psychology, special education) plus two years of relevant experience, or a Master’s degree plus one year of experience, or an Associate's degree with four years of experience.
  • Preferred Skills: Bilingual abilities in Spanish or Nepali are a plus.
  • Strong communication, organizational, and interpersonal skills.
  • Valid PA driver’s license and an operable, insured vehicle is required.
  • Must pass Act 33 and Act 34 clearances, along with FBI background check.
