Job Details - RN Case Manager Home Health

• This position will be in the field, visiting patients in their homes
• You can start your day from your home and patients will be scheduled closest to your home (as best as possible)
• The RN will provide wound care, med administration, IV therapy and more

Schedule: 8am - 5pm | M-F

Home Health RN Case ManagerRequirements:

• Current good standing RN license in State of PA
• 1+ Years of experience in Home Health, SNF, Med-Surg, ED or ICU
