Overview:

Sentara RMH Medical Center is now hiring a Medical Social Worker. This is a position that will be responsible for the provision of medical social work services to patients receiving care in the hospital.

Education

Master’s Degree – Social WorkĀ  (Required)

Certification/Licensure

  • Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hireĀ  (Required)
  • Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hireĀ  (Required)

Experience

  • 1 year of case management experience (Required)
  • 1 year of acute care experience (Required)
  • Experience with discharge planning (Preferred)

Duties/Responsibilities

  • Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.
  • Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.
