Overview
At the direction of the Clinical Manager, coordinates/manages a case load comprised of Hospice patients. Â Assesses, plans, implements, coordinates, and evaluates care for each patient/family in conjunction with the hospice interdisciplinary team. Â Case management includes but is not limited to; comprehensive needs assessments, development of appropriate plan of care, referral to internal and external resources, evaluation of outcomes, and provision of nursing educational, clinical and related services. Â Supervises the Licensed Nursing Assistants/ Home Health Aides.

Qualifications
REQUIRED QUALIFICATIONS
Â
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing

PREFERRED QUALIFICATIONS
Â
- Minimum two years’ general experience in a supervised nursing setting recommended.Â
- Hospice/Homecare experience preferred.
- Certified in Hospice/Palliative Care Nursing desirable.
Required Licensure/Certifications
Â
- Unencumbered RN license in VT and NH or compact.
