POSITION SUMMARY:

 

The Transitional Care Management Nurse supports Medicare patients after hospital discharge through assessment, planning, facilitation, and advocacy to promote effective, cost‑efficient outcomes. This role assists patients in understanding and using their health plan benefits, coordinates needed services and works to remove barriers to appropriate care. The nurse functions independently and collaboratively within a multi‑specialty clinic environment and provides interactive contact and non–face‑to‑face services for recently discharged Medicare patients. All duties are performed in alignment with the Mission and Values of Hattiesburg Clinic.

 

 

EDUCATION & EXPERIENCE:

 

  • Graduation certificate from an accredited Registered Nurse (RN) program, required
  • Current MS state licensure, required
  • CPR, ACLS, and IV certifications, preferred
  • Three (3) years of prior experience, preferred

  • Knowledge of EMR, medical management and policies and procedures
  • Intermediate skills in work processing, spreadsheet and database development and maintenance

 

 

WORK SCHEDULE:

 

Monday - Friday, 8:00AM - 5:00PM.  Position may transition to a hybrid role, after training is complete.
