Licensed Clinical Social Worker - Quality Management

POSITION SUMMARY:

The Quality Management Licensed Clinical Social Worker is responsible for assisting Medicare patients enrolled in the Case Management, Value Based Care programs and their families in dealing with personal, social, emotional, financial and environmental problems which result from their illness or treatment. The Social Worker will coordinate, develop and act as a resource to assist with Social Drivers of Health identified by a patient's care team.

QUALIFICATIONS:

Education needs to be Master of Social Work (MSW) from a CSWE-accredited program, required. Current Mississippi LCSW license required; willingness to improve social work skills through a personal plan of continuing education; ability to use a variety of physical motions in day-to-day activity including standing, turning, bending, stooping, lifting, and climbing; ability to stand and walk for extended periods of time without experiencing fatigue; ability to accept responsibility and delegate duties; ability to access patient psychosocial needs and coordinate with other staff members; professional in relating to the other medical staff and other departments; ability to keep all information regarding patients and/or families confidential.

WORK SCHEDULE:

Monday - Friday, 8:00AM - 5:00PM
