Job Summary

Our Mass General Brigham Medical Group community healthcare center in Haverhill located at 1 Park Way in Haverhill has the latest in diagnostics, technology, and research. We proudly offer our patients unparalleled acute, chronic, and wellness care. Whether you visit us once a year for a physical or more often for a chronic health problem, our team will provide you and your family with the service, information, and education required to achieve and maintain good health. Our multi-specialty group practice offers a broad range of medical sub-specialties.

We are seeking a full-time 40-hour Diabetes Program Coordinator to support our Internal Medicine suite Monday through Friday from 8:00-5:00pm. The schedule consists of 4 days in Haverhill and 1 day at our Andover location.

Summary
Responsible for overseeing the successful planning and completion of educational programs for Patient care.

Essential Functions
-Maintain accreditation for the program.
-Conduct individual and group patient education sessions; maintain the schedule of patient appointments.
-Coordinate reporting for audits and implement recommendations as a result of the audits.
-Maintain records pertaining to program activities.
-Purchase or approve the purchase of equipment and supplies needed by the staff and program.
-Schedule program work, oversee daily operations, coordinate the activities of the program, and set priorities for managing the program.
-Manage marketing and outreach to the community.
-Prepare or assist in the preparation of grant requests and funding continuation from outside sponsors.

Education
Bachelor's Degree Related Field of Study required
