Position: Wraparound Facilitator

Location: Any HPMHC office location (Hays, Norton, Phillipsburg, Colby, Goodland, Osborne)

Pay: $22.36 per hour | $500 SIGN-ON BONUS!

Job Type: Full-Time | In-Person

Schedule: typical hours Monday- Friday 8am-5pm

Responsibilities Include:

  • Facilitating Wraparound planning meetings with families and support teams
  • Helping families navigate the SED/HCBS Waiver and PRTF Diversion Grant programs
  • Coordinating services and community resources to support treatment goals
  • Developing individualized plans that focus on strengths, needs, and long-term success
  • Collaborating with families, educators, providers, and community partners
  • Monitoring progress and adjusting plans to ensure positive outcomes
  • Advocating for children and families within schools, agencies, and community systems
  • Completing documentation and maintaining compliance standards
  • Traveling within the service area to meet with families and community partners

Requirements

  • Experience counts! We accept equivalent work experience in human services (teaching, daycare, CNA, etc.) or a BA/BS degree. One year of related work = one year of college.
  • Great with people—strong communication and empathy are a must. Experience working in human services, behavioral health, social services, education, or a related field preferred.
  • Comfortable in a variety of human service settings. Must be able to work effectively with children experiencing Severe Emotional Disturbance (SED) and their families.
  • Able to pass drug screen and required background checks.
  • Must have valid driver’s license.
