About the Role:
Join Skilled Nursing Pharmacy as a Staff Pharmacist in Hayward, CA, where you will play a vital role in providing exceptional pharmaceutical care to our patients. Responsibilities:
- Dispense medications accurately and in a timely manner, ensuring patient safety.
- Conduct medication therapy management and patient consultations.
- Collaborate with healthcare providers to optimize patient medication regimens.
- Monitor patient medication therapies and identify potential drug interactions.
- Maintain accurate and up-to-date patient records and medication profiles.
- Provide education and support to patients and their families regarding medications.
- Participate in quality assurance initiatives and adhere to regulatory standards.
- Assist in training and mentoring pharmacy technicians and interns.
Requirements:
- PharmD degree from an accredited pharmacy school.
- Active California pharmacist license in good standing.
- Minimum of 1 year of experience in a pharmacy setting, preferably in long-term care.
- Strong communication skills and a patient-centered approach.
- Ability to work collaboratively in a fast-paced environment.
- Knowledge of pharmacy software and electronic health records.
- Commitment to continuous learning and professional development.
- Detail-oriented with strong analytical and problem-solving skills.
