Position: Community Support Associate/Life Skills Coach/Transporter

Company/Location: Kentucky River Community Care/Breathitt County Health & Wellness, Hazard, KY

Employment type: Regular Full-Time

Credentials: High School diploma or GED with one year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder.  Must have valid driver's license with 2 years of driving experience.

Special skills: Conscientious, Dependable, Good computer skills, Works well with other staff, Good communication skills.  Ability to work independently.  Good organizational and time management skills.

Primary functions:

  • Teaching clients the following skills: how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
  • Complete documentation in EMR within 24 hours of service delivery.
  • Conduct activities necessary to allow individuals to live with maximum independence in the community.
  • Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
  • Maintain all trainings and required supervision.
  • Participate in therapeutic rehabilitation and 1915i Rise services as needed.

Shift/Days & Work Hours: Monday - Friday 8:00AM - 4:00PM

Funding Source: Meet requirements 908 KAR 2250, Billable for H2015, Medicaid.
