Description

     Shodair is looking for a Float Patient Care Coordinator to join the team! The Float PCC does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs. The Float Patient Care Coordinator provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service. 


What You'll Do:

  • The Float PCC does not have an assigned patient caseload; however, the position supports Acute, Residential and Resource Patient Care Coordinators with coverage needs. 
  • Provides skills coaching to patients in a group setting at least 4 times weekly and participates in 1:1 care coordination service.
  • Coordinates care to patients by developing, monitoring, and coordinating interdisciplinary care in conjunction with the treatment team. 
  • Provides effective leadership and advocacy in building relationships between teams, families, and other community professionals. 
  • Completes their own billing and note documentation to comply with hospital and governing agency standards.
  • Assists families in navigating community services and systems and can help in the application and referral process.

This position starts at $23.65 per hour!

Requirements



EDUCATION, TRAINING AND EXPERIENCE

  • Post-secondary education or Bachelor’s degree in a human services field strongly preferred.
  • At least 3 year of experience serving people with mental illness is required.
  • 5 years of experience in case management or patient care in a healthcare or social services delivery system preferred. 1 Year experience in a patient care setting.
