The OB Nurse Navigator serves as a dedicated, compassionate advocate for patients throughout their pregnancy journey, providing care coordination, education, and support from initial OB visits through postpartum. This RN role ensures seamless communication across multidisciplinary teams, assists with scheduling, triages patient concerns, and enhances care quality and satisfaction.

Key Responsibilities:

  • Guide patients from OB confirmation through delivery and postpartum.

  • Coordinate care plans, prenatal appointments, labs, C-sections, and referrals.

  • Triage calls and messages; support patients experiencing miscarriage or postpartum needs.

  • Educate on prenatal health, nutrition, warning signs, discharge planning, and available resources.

  • Conduct screenings (EPDS, SDOH, substance use) and administer treatments such as NSTs.

  • Maintain accurate EHR documentation and ensure smooth clinic operations.

  • Participate in Baby Friendly Program and continuous quality improvement initiatives.

  • Support exam room readiness and assist with training or education as needed.

  • May float to other areas within SPHMG as required.

Qualifications:

  • RN license in Montana; BLS certification required.

  • Graduate of accredited nursing program (BSN preferred).

  • Minimum 1 year OB nursing experience; L&D experience preferred.

  • Strong triage, coordination, technology, and communication skills.
