The Staff Pad has partnered with a leading healthcare organization in Helena, Montana, to hire a compassionate and experienced Occupational Therapist for its Home Health team.

In this role, you'll deliver skilled, patient-centered occupational therapy services that help patients regain independence, improve daily function, and enhance their overall quality of life.

Reporting to the Director of Rehabilitation, you will evaluate patients, develop individualized treatment plans, provide care in the home setting, and collaborate with an interdisciplinary team to ensure exceptional clinical outcomes and regulatory compliance.

What You'll Do

Key Responsibilities

  • Evaluate patients and develop individualized occupational therapy treatment plans
  • Provide skilled OT services in patients' homes to improve function and independence
  • Complete OASIS assessments and clinical documentation accurately and on time
  • Educate patients and caregivers on daily living skills, home safety, and adaptive techniques
  • Conduct home safety assessments and recommend adaptive equipment
  • Develop personalized home exercise and treatment programs
  • Collaborate with physicians, nurses, therapists, and home health aides
  • Support patients with a variety of diagnoses, including neurological conditions, chronic illnesses, and cognitive impairments
  • Coordinate care to ensure continuity and quality patient outcomes
  • Participate in case conferences, staff meetings, and continuing education

What We're Looking For

Education, Licensure, Certifications and Experience

  • Graduate of an accredited Occupational Therapy program
  • Current Montana Occupational Therapist license & BLS certification
  • 2+ years of experience in Acute Care, Outpatient, Rehabilitation, Home Health, or related setting
  • Experience with OASIS documentation preferred
  • Home Health or Home and Community Based Program experience is a plus
