Job DetailsJob Location: Henderson, KY 42420Position Type: Full TimeEducation Level: Bachelor's degreeSalary Range: $24.00 - $27.00 HourlyTravel Percentage: OccasionalJob Shift: DayThe Care Manager – Homeless Outreach Specialist provides community-based outreach and housing stabilization services to individuals experiencing homelessness who have mental health and/or substance use needs in Webster and Henderson Counties, with an emphasis on Veterans. Funded through the PATH Grant and SOAR Grant, this position connects people to MH services, benefits and supportive services, utilizing Care Management practices to engage individuals who are unsheltered or disconnected from services. The PATH-SOAR Care Manager supports access to SOAR-based SSI/SSDI benefits, peer support services, and integrated behavioral and physical health care using a person-centered, and trauma-informed approach. The role emphasizes engagement, systems navigation, housing/shelter, and service stabilization through coordinated care. This position will provide coverage across Henderson and Webster counties. Essential Functions:  Conduct proactive outreach in streets, shelters, encampments, hospitals, Veterans’ services and organizations, and other community-based settings Works closely with outreach teams, shelters, law enforcement, and homeless coalitions to promote housing stability and improvement in quality of life. Engage individuals with significant behavioral health needs or substance use using trauma-informed, person-centered, and non-coercive approaches Establish and maintain trusting, ongoing relationships with individuals disengaged from traditional services Screen for mental health and substance use needs and coordinate access to clinical assessments and treatment Provide brief interventions, psychoeducation, and crisis support within scope of role and facilitate engagement with crisis services Support medication access and coordination with prescribing providers Utilize harm-reduction strategies to reduce risk and support participant-identified goals Support rapid access to transitional housing while pursuing permanent supportive housing options in collaboration with the In-Reach Housing Coordinator Assist participants with housing applications, documentation, and move-in coordination Provide ongoing tenancy support and link participants to payee services as needed Participate in multidisciplinary team meetings and advocate for participants across service systems Complete required documentation accurately and timely and track program outcomes, completes monthly reports to be submitted for the grants. Meets benchmarks for outreach activities as required by the grants. Other duties as assigned   QualificationsEducation and Experience:  Bachelor’s degree in social work, psychology, human services, or related field. One year post graduate experience preferred. Experience working with individuals experiencing homelessness and serious mental illness, substance use disorders, or co-occurring conditions Strong communication, engagement, and de-escalation skills Ability to work independently in community-based settings Valid driver’s license and reliable transportation if required   Knowledge and Skills:  Valid driver’s license and the ability to provide safe and reliable transportation. Good oral and written communication skills. Physical ability to do light lifting. All Case Managers are required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually. Complete Case Management Training (conducted by DDID or ABIB), CDS modules, CPR/First Aid, and Medication Administration/seizure training before beginning independent functioning as a Case Manager.
