Role Overview: Licensed Clinical Social Worker - Part Time - Richmond, VA

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our Richmond team on a part-time basis. This role provides the opportunity to deliver person-centred psychosocial care within post‑acute, long‑term care and assisted living settings. The successful clinician will work collaboratively with interdisciplinary teams, support residents and families, and contribute to continuity of care while maintaining flexible scheduling and professional autonomy.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments for admissions, routine reviews and as‑needed evaluations

  • Develop and implement individualized treatment and discharge plans in collaboration with facility staff and families

  • Provide brief therapy, crisis intervention, grief and bereavement support, and behavioral health management appropriate to the post‑acute setting

  • Coordinate community resources, referrals and social supports to promote resident wellbeing and safe transitions

  • Participate in interdisciplinary care conferences, care planning meetings and family consultations

  • Maintain accurate, timely documentation in the Electronic Medical Record (EMR), ensuring clear communication of plans and follow‑up

  • Contribute to quality improvement initiatives, case reviews and infection prevention activities as required

  • Provide education and clinical guidance to facility staff, residents and families on psychosocial issues and care approaches

Qualifications and Required Skills

  • Master’s degree in social work (MSW) from an accredited institution

  • Active, unrestricted Virginia LCSW license and current malpractice coverage as required

  • Experience in post‑acute, long‑term care, geriatrics, behavioral health or related settings preferred

  • Strong assessment, care planning and brief therapy skills with sensitivity to older adults and diverse populations

  • Proficiency with Electronic Medical Record systems and timely, clear documentation practices

  • Excellent communication, interdisciplinary collaboration and problem‑solving abilities

  • Culturally sensitive, person‑centered approach and commitment to preserving dignity in care
