We are seeking compassionate full-time and part-time Licensed Practical Nurses (LPNs) to join our Henryetta care team. Working collaboratively with our supervising Registered Nurses, your expertise will make a significant impact on the lives of those we serve by helping local patients safely manage chronic conditions and achieve their optimal health at home.

Position Details:

  • Full-Time: Approximately 25–30 visits per week
  • Part-Time: Variable schedule based on patient needs and availability
  • Compensation: $43.00–$45.00 per visit

Key Responsibilities:

  • Provide skilled nursing care in the home as ordered on the plan of treatment and nursing care plan.
  • Evaluate the significance of assessment findings and communicate pertinent information about the patient's clinical status and ongoing needs to the care team in a timely manner.
  • Adequately assess and reassess pain, utilize appropriate pain management techniques, and educate patients and families regarding pain management.
  • Maintain accurate medical documentation and update medical records in a timely manner.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Educate patients and their families about health management and disease prevention.

Requirements:

  • Current LPN license in the state of Oklahoma in good standing.
  • Graduate of an accredited practical nursing program with current CPR/BLS certification.
  • Knowledge of pain management techniques and patient care best practices.
  • Familiarity with medical documentation practices and electronic health record (EMR) systems is preferred.
  • Strong interpersonal skills with the ability to build rapport with patients and families.
  • Valid driver's license and reliable transportation for local home visits.
