Position Summary
• Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.

Education
• Masters Degree in Physical Therapy or Bachelor’s degree in Physical Therapy with a minimum 5 years of work experience.

Licenses and Certification
• Illinois license in Physical Therapy
• BLS

Experience and Skills
• Technical Experience: N/A

Role Specific Responsibilities
• Performs thorough evaluation
• Develops appropriate Plan of Care
• Implements Plan of Care
• Documentation is accurate, complete and timely
• Provides supervision to assistants, rehab techs, and students
