Job DetailsJob Location: Hiawatha Community Services Building - Hiawatha, KS 66434Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $20.00 - $28.00 HourlyJob Shift: DayPURPOSE: The Case Manager Facilitator (Wraparound Facilitator) provides comprehensive support and coordination for children and families enrolled in the SED Waiver program. The goal is to ensure youth with complex behavioral health needs receive individualized, community-based services that promote overall well-being and success across life domains. This role also works closely with insurance providers to coordinate, develop, and maintain effective plans of care. Essential Job Functions Collaborate with families, guardians, foster care agencies, schools, and other identified supports. Facilitate regular meetings with the youth’s treatment team, including mental health professionals, educators, probation officers, and community partners. Coordinate referrals and facilitate the initial SED Waiver enrollment process. Work closely with insurance providers to develop, coordinate, and maintain individualized plans of care. Educate team members and external partners on waiver services and program requirements. Ensure compliance with Medicaid guidelines for service delivery and documentation. Maintain accurate, timely, and confidential records of client progress, services, and interventions. Monitor implementation of care plans and evaluate effectiveness in achieving desired outcomes. Reconvene the wraparound team at least every 90 days and annually to review and adjust care plans. Document changes in client needs and update care plans accordingly. Manage a small caseload and provide ongoing case management and waiver services. Perform other duties as assigned. Required Knowledge, Skills, and Abilities: Bachelor’s degree in human services or related field preferred, or equivalent combination of education and experience (minimum of four years in human services). Must be at least 21 years of age. Ability to pass pre-employment background check and drug screening. Must complete required SED Waiver, case management, and crisis training within 60 days of hire, as required by the State of Kansas. Experience working with individuals with SMI, SED, or SUD preferred. Strong organizational and time management skills. Excellent verbal and written communication skills. Ability to work independently and as part of a team. Proficiency with computers and electronic health record systems. Ability to maintain professional boundaries and confidentiality. Valid driver’s license with acceptable driving record; ability to safely operate a company vehicle. Physical Demands Ability to remain stationary (sitting or standing) for extended periods. Frequent movement between worksites. Ability to lift or move items up to 10 pounds. Repetitive use of hands, wrists, and fingers. Ability to communicate effectively to exchange information. Ability to work in varying outdoor conditions, including temperature and weather changes. Working Environment Work is performed in a variety of settings, including office environments, schools, and client homes. Duties are typically performed indoors with standard office equipment and conditions; however, travel to multiple worksites is required. This position involves use of a company vehicle and may include exposure to diverse home environments and occasional high-stress or potentially aggressive situations.
