Fairview Range is hiring a Social Worker - MSW to join our Home Care and Hospice Team in Hibbing, MN.

As a Social Worker in this role, you will oversee direct service to Hospice and Home Care clients and families through collaboration with the interdisciplinary team and resource agencies. The Social Worker will also provide support and/or supervise the hospice bereavement program.

Social Worker Responsibilities
- Ability to assess and plan for interventions related to patient and family needs and behaviors in compliance with current policy and procedures.
- Demonstrates knowledge of hospice program criteria and ability to explain to patient/family, referral sources, and hospice staff.
- Advocates for patient/family.
- Demonstrates and communicates understanding of Hospice coverage under Medicare, Medicaid, and Private Insurances. Obtains Prior Authorization as needed. Communicates with case manager as indicated. Works with patient/family and community resources as needed for financial concerns.
- Works closely and proactively with RN case manager for all aspects of hospice care. Demonstrates working knowledge/understanding of pharmacological and non-pharmacological pain and symptom management.
- Demonstrates ability to effectively provide needed support to hospice team, patient, and families.
- Demonstrates knowledge of DPOA, Living Wills/Directives to Physicians. Ensures clients have needed documents in place to assist in decision making.
- Assesses decision-making capacity of patient and patient representatives on ongoing basis and care plans as indicated.
- Proactively assists with funeral planning and communicates to hospice team.
Required Qualifications
- Master of Social Work
- Current MN Social Worker licensure
- Current MN driver’s license and auto insurance
