MDS Coordinator (RN) – Full-Time
Hickory, NC

We are seeking an experienced and detail-oriented MDS Coordinator (RN) to join our team in Hickory, NC. This is a great opportunity for a nurse who thrives in a collaborative environment and has a strong understanding of the MDS process, reimbursement, and regulatory compliance.

Position Summary

The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments, ensuring accuracy, timeliness, and compliance with all federal and state regulations. This role plays a key part in supporting quality outcomes and appropriate reimbursement.

Key Responsibilities

  • Complete and oversee MDS assessments (OBRA, PPS, and PDPM) in an accurate and timely manner 
  • Ensure compliance with CMS guidelines and state regulations 
  • Coordinate with interdisciplinary team members to gather necessary documentation 
  • Monitor case mix index (CMI) and identify opportunities for optimization 
  • Participate in care plan meetings and ensure care plans reflect resident needs 
  • Conduct audits to ensure documentation supports coding and reimbursement 
  • Stay current on regulatory changes and best practices 

Qualifications

  • Active Registered Nurse (RN) license in North Carolina 
  • Previous MDS experience in a skilled nursing or long-term care setting required 
  • Strong knowledge of PDPM, Medicare, and Medicaid reimbursement 
  • Excellent attention to detail and organizational skills 
  • Ability to work collaboratively with clinical and administrative teams 
  • Strong communication and problem-solving skills
