Sage Group HealthCare Staffing is seeking PT Home Health for 13-week travel/contract positions in Columbus, OH. We are accepting both local and traveler candidates.

Pay Rate: Range $2560 to $2640 Weekly Gross pay based on a 40 hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health

Shifts available: Days

Shift timings: 8 Hours; Days, 8.00a - 5.00p

Schedule: 40 hours/week guaranteed, 5 X 8-hr shifts per week, Mon-Fri, 32 productivity points per week expected

Position Requirements:

Current PT experience: 1- 2 years - Required
PT Home Health experience: 1 year within the last 3 years - Required
Home Care Home Base (HCHB) charting experience – Required
OASIS experience - Required
Must be Covid Vaccinated or provide medical or religious exemption

Education, License and Certification Requirements:

Bachelor of Science in Physical Therapy or Similar - Required
Current State License as Physical Therapist or Compact if applicable - Required
BLS Certification from AHA or American Red Cross - Required

Job Description:

Provide skilled physical therapy services to patients in their homes as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to IDT team
Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
Oversee and supervise therapy services of patient provided by PTAs and when appropriate, nurse assistants.
Communicate with the PTAs and nurse aides on supervisory visit results.
Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Must meet assigned productivity points per week
Territories to cover: Columbus, OH and surrounding areas
