The Clinical Pharmacist- Ambulatory Care is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients.聽 In addition, will serve as a resource to the pharmacy, nursing and medical staff within the field of primary care drug management.

KEY RESPONSIBILITIES
Performed majority of the time:
路聽聽聽聽聽聽 Provides collaborative drug therapy management to patients in the primary care clinic to include:
o聽聽 Initiation and dose adjustment of medications for conditions outlined in the Physician/APP orders.
o聽聽 Patient and family education.
o聽聽 Drug/drug interaction monitoring.
o聽聽 Drug/food interaction monitoring.
o聽聽 Follow-up instructions.
o聽聽 Coordination with diabetes education, nutrition, home health services and other appropriate resources as applicable to assure optimum patient outcomes.
o聽聽 Assist in appropriate patient screenings and follow-up for depression and substance use.
o聽聽 Appropriately documents all clinical activities.
o聽聽 Acquires any needed prior authorizations.
路聽聽聽聽聽聽 Coordinates pharmaceutical care across all ambulatory care clinics:
o聽聽 Coordinates medication-therapy related quality initiatives.
o聽聽 Develops clinical workflows.
o聽聽 Develops and manages outpatient medication formulary for primary care clinics.
路聽聽聽聽聽聽 Participates in the training of practitioners, pharmacists, other healthcare professionals, and learners (including pharmacy and medical students/residents); preceptor for pharmacy residency program.
Performed occasionally but critical to successful performance of the job:
路聽聽聽聽聽聽 Serves as a resource to the pharmacy, nursing, and medical staffs in primary care.

o聽聽 Completion of a PGY-1 residency plus 2 years of ambulatory care experience.
o聽聽 Completion of a PGY-2 ambulatory care residency.
o聽聽 5 years of experience in ambulatory pharmacy care.
Preferred
路聽聽聽聽聽聽 Comprehensive clinical pharmacy knowledge base with emphasis in the field of depression and substance use.
Licenses, Certifications and/or Registrations:
Required
路聽聽聽聽聽聽 Current, valid pharmacy license by the Oregon Board of Pharmacy.
路聽聽聽聽聽聽 BLS required within 90-days of hire.
路聽聽聽聽聽聽 ACLS required within 90-days of hire.
Preferred
路聽聽聽聽聽聽 Active board-certified ambulatory care pharmacy specialty certification.
