KEY RESPONSIBILITIES:
路聽聽聽聽聽聽聽 Responsible for taking and processing Palliative Care referrals.
路聽聽聽聽聽聽聽 Provide support to the Palliative Care Nurse Practitioner and Medical Director.
路聽聽聽聽聽聽聽 Will schedule visits to support goals of care as ordered by MD.
路聽聽聽聽聽聽聽 Coordinate and communicate patient care goals between all providers.
路聽聽聽聽聽聽聽 Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.
路聽聽聽聽聽聽聽 Ensure communication of assessment and reassessment findings to attending physician.
路聽聽聽聽聽聽聽 Complete all documentation accurately, timely, and in accordance with company policy and procedure.
路聽聽聽聽聽聽聽 Inform palliative team of all new admissions and discharges.
路聽聽聽聽聽聽聽 Coordinate referrals utilizing the referral process.
路聽聽聽聽聽聽聽 Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.
路聽聽聽聽聽聽聽 Run billing report and address validation holds/ billing errors

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:
路聽聽聽聽聽聽聽 Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN)聽licensure in the state of practice
路聽聽聽聽聽聽聽 A minimum of three (3) years of nursing experience
路聽聽聽聽聽聽聽 Palliative care, hospice, or home health care experience is preferred
路聽聽聽聽聽聽聽 CHPN (Certified Hospice and Palliative Nurse) is preferred
