GiaCare is currently seeking a full-time Mental Health Registered Nurse Case Manager to support the 49th Medical Group at Holloman Air Force Base in Alamogordo, New Mexico.
About the Opportunity
This position offers experienced Registered Nurses the opportunity to provide comprehensive case-management services to members of the military community in a collaborative outpatient environment. The RN Case Manager will coordinate care across inpatient, outpatient, community, and telephonic settings while supporting continuity of care and optimal patient outcomes.
Schedule
- Full-time, Monday through Friday
- Eight working hours scheduled between 7:00 AM and 5:00 PM
- One-hour unpaid lunch
- No weekends
- No on-call requirements
- Federal holidays off
- Schedule provided approximately one month in advance
Compensation and Benefits
- $51.61 per hour
- Additional $4.57 per hour in health and welfare benefits

Responsibilities
- Assess patient needs and develop, implement, coordinate, and monitor individualized care plans
- Provide case-management services through inpatient, outpatient, onsite, community, and telephonic settings
- Coordinate treatment plans that include preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions
- Support continuity of care and follow-up following discharge
- Integrate nursing case management with utilization management and social-work case management
- Collaborate with physicians, nurses, social workers, caregivers, community organizations, and other members of the healthcare team
- Identify appropriate healthcare and community resources for patients
- Maintain accurate patient-care records and case-management documentation
- Provide health education and instructions to patients and caregivers based on identified needs
- Assist in developing and implementing patient-assessment tools, clinical guidelines, algorithms, case-management processes, and community-resource databases
- Participate in quality-improvement activities, meetings, training, and other approved administrative functions
Minimum Qualifications
- U.S. Citizen, U.S. national, lawful permanent resident, or individual otherwise authorized to work in the United States
- Associate's Degree of Nursing
- Current, full, active, unrestricted Registered Nurse license in any U.S. state, territory, or the District of Columbia
- At least one year of professional nursing experience following graduation
- Current Basic Life Support (BLS) certification (or willing to obtain)
- Demonstrated clinical competency within the previous two years
Case Management Certification
Candidates who do not currently possess an approved case-management certification may still qualify but must obtain one within the first 12 months of employment.
