Hours: PRN

SUMMARY

The Chaplain provides spiritual counsel and support to the hospice patients and their families.


ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Completes an initial spiritual assessment of patient's and family’s needs according to agency standards and regulatory requirements.
  2. Develops and implements a plan of care with members of the interdisciplinary team, the patient and family; and includes the patient's faith community if requested.
  3. Visits patients and families, providing spiritual counsel and care in a manner accessible to and agreeable to the individual patient and family.
  4. Engages in prayer and sacramental ministry to patients and families, as requested, and as appropriate.
  5. Plans and conducts funerals or memorial services, as requested by patients and/or families.
  6. Assists with patients/families in crisis.
  7. Communicates with patient's clergy or faith community as appropriate.
  8. Provides bereavement support and counsel for patients, families and staff
  9. Educates staff on beliefs and rituals of different faiths.
  10. Participates in interdisciplinary care planning conference and team meetings.
  11. Provides community education, as requested, particularly in relation to local clergy and church groups.
  12. Completes clinical documentation in a timely manner per Alive Hospice procedure and regulations
  13. Facilitates services in a patient room, home or quiet room-as requested.
  14. Attends department and discipline meetings and in-services as scheduled.
  15. Participates in memorial services and agency events as requested or assigned.
  16. Participates in the on call schedule rotation.
  17. Other duties may also be assigned.

Requirements

EDUCATION and/or EXPERIENCE

Master of Divinity Degree or equivalent graduate degree that provides advanced education to provide spiritual care and counseling. Minimum of one (1) unit of Clinical Pastoral Education (CPE) required. 


CERTIFICATES, LICENSES, REGISTRATIONS

Current driver's license and automobile insurance as required by Tennessee State Law.
