Title: Social Service Director
Department: Social Services/Administration
Location: Symphony Care Network
Status: Full 


Position Summary: In keeping with our organization’s goal of improving the lives of the Guests we serve, the Social Services Coordinator ensures that the medically related emotional and social needs of the Guests are met and maintained on an individual basis and in accordance with current federal, state, and local regulations. 


Essential Functions:

  • The social worker to appropriately transition the resident and family through the care process
  • Access cognitive capacity and mood assessments utilizing standard tools
  • Implement healthcare directives and ability to have discussions and knowledge of end-of-life care and hospice
  • Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician


Requirements

 Qualifications/Experience:

  • MUST have minimum2-3 years’ experience in long term care and and skilled nursing care
  • Must have a Bachelors in social work
  • Must demonstrate strong conflict resolution
  • Therapeutic relationship skills
  • Insurance/Medicare proficiency  
  • Responsible for keeping up-to-date evaluation documentation on each Guest’s activities at the facility which complies with Federal, State, and Local regulations
  • Coordinates services with psychiatric providers
  • Develops a comprehensive discharge plan in collaboration with Guests, Guests' families, multidisciplinary staff, and referral agencies  
  • Excellent time management skills with a proven ability to meet deadlines
