WMCHC’s Mission Wayne Memorial Community Health Centers creates and sustains healthy rural communities by providing accessible, high quality medical, dental, and behavioral health care for all, while recognizing and addressing other factors that influence health outcomes.  Job Responsibilities: The Community Health Worker and Outreach and Enrollment Coordinator plays a vital role in our Federally Qualified Health Center (FQHC) by bridging the gap between clinical care and social determinants of health. This position proactively connects patients with critical social services, insurance options, and community resources to ensure all physical and mental health needs are met without barriers. As a trusted advocate, the Coordinator conducts direct community outreach, guides individuals through complex healthcare enrollment systems, and provides continuous navigation support to improve overall health outcomes for underserved populations. QualificationsQualifications: High school diploma or equivalent required; Associate degree in a healthcare-related field preferred. Current Certified Community Health Worker (CCHW) designation or the ability to successfully complete training within 6 months of hire. Ability to successfully complete all PENNIE Assister Training Modules and pass certification exam upon hire. Proficiency in computer skills to include but not limited to online portals (PENNIE, Medicaid/ COMPASS), Microsoft Excel, Electronic Health Records.
