What You Bring

  • Credentials: Active MA, LPN, or RN license/certification in the state of Hawaii
  • Technical: Computer savvy; intermediate proficiency in MS Suite — Word, Teams, Outlook, and Excel
  • Communication: Strong verbal and written communication skills; professional phone etiquette and excellent customer service
  • Organization: Excellent attention to detail, strong time management, and ability to prioritize time-sensitive situations with urgency
  • Interpersonal: Strong interpersonal skills with the ability to communicate effectively across a variety of people and settings
  • Adaptability: Ability to multitask while providing compassionate patient care in a fast-paced environment
  • Leadership: Leadership qualities with the ability to work well within a team of healthcare professionals

What You’ll Do

  • Maintain care coordination dashboards, patient lists, and tracking for CCM/PCM programs
  • Track referrals, authorizations, and follow-up appointments across specialty and post-acute services
  • Conduct non-clinical patient and family outreach, including welcome calls and appointment reminders
  • Coordinate communication between patients, facilities, Care Managers, and providers
  • Gather, upload, and organize medical records, wound photos, labs, and imaging reports
  • Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments
  • Complete clerical tasks related to Care Management, including outreach, scheduling, and documentation
  • Monitor admissions, discharges, and census changes from all care settings
  • Prepare patient summaries, reports, and materials for care team meetings
  • Escalate clinical concerns promptly to a Care Manager or provider
  • Participate in required training, meetings, and compliance activities
