This role involves assessing patient needs, implementing care plans, and collaborating with the healthcare team to deliver comprehensive wound management. Key Responsibilities: - Conduct thorough assessments of patients' wound conditions and document findings accurately. - Develop and implement individualized wound care plans in collaboration with the healthcare team. - Perform wound care procedures, including cleaning, dressing, and monitoring wounds, following established protocols. - Educate patients and their families on wound care techniques and prevention strategies. - Monitor and evaluate patients' progress, adjusting care plans as necessary to achieve optimal outcomes. - Maintain accurate and timely documentation of patient care activities and progress. - Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care. - Stay updated on the latest wound care techniques and best practices through continuous education and training. - Adhere to all healthcare regulations and standards to ensure patient safety and quality of care. - Participate in quality improvement initiatives to enhance wound care services and patient satisfaction. QualificationsRequired Education: - Completion of a state-approved Licensed Practical Nurse (LPN) program. - Current and valid LPN license in the state of practice. Required Experience: - Minimum of 2 years of experience as an LPN, preferably in a wound care or similar healthcare setting. - Proven track record of working effectively in a multidisciplinary healthcare team. - Experience in assessing, planning, and implementing wound care treatment plans. Required Skills and Abilities: - Strong knowledge of wound care practices, including infection control and prevention techniques. - Excellent communication skills to effectively interact with patients, families, and healthcare team members. - Ability to perform wound assessments and document findings accurately. - Proficient in the use of electronic health records (EHR) for documentation and patient care management. - Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment. - Compassionate and patient-centered approach to care, with a focus on improving patient outcomes. - Ability to maintain confidentiality and adhere to ethical standards in all aspects of patient care.
