Overview:Â
Resident Care Team Leader Licensed Vocational Nurse will assist in ensuring general health and well-being of our residents by providing direct nursing care. This position is responsible for the direct nursing care of residents which encompasses planning, developing, organizing, implementing, and evaluating the care of residents to ensure that the highest degree of quality care and regulatory compliance is always maintained.Â

Duties and Responsibilities:Â
- The Resident Care Team Leader heads the Resident Care Team, which consists of the Leader, a Resident Care Assistant and volunteer for direct care givers.Â
- Participates in the direct and indirect care of residents and ancillary hospice services to residents’ family and friends.Â
- Performs all nursing duties based on standard of care established by the State Board of Nursing.Â
- Oversees implementation of the plan of care, nursing interventions, direct nursing care distribution of medication, documentation of all care proved and bereavement for all residents.Â
- Inventory of controlled substances, documentation of dispensed controlled substances.Â
- Responsible for ensuring that the shift team meets all resident needs for personal care and ADL’s, activity, stimulation, mobility, safety, hydration, nutrition, elimination, and post-mortem care for all residents.Â
- Coordinates and maintains a clean and safe environment for residents, oversees meal and snack preparedness.Â
- Participates in Quality Assurance projects.Â
- Based on experience may be asked to assist as ‘on call’ staff.Â
- Performs other duties as assigned by the Director of Nursing Services.Â
Â
Education, Licensure/Certification:Â Â
- Currently licensed as a Registered nurse (RN) or Licensed Vocational Nurse (LVN) in good standing with the State of Texas.Â
- Licensed in the State of Texas.Â
- BLS Certification required. Â
Â
Experience, Skills/Abilities Related Requirements:Â
- Can work to prescribed nursing standards including the physical demands of standing, walking, and moving most of the time without lifting restrictions.Â
- 1-year acute care experience required.Â
- 1 year Hospice and/or HIV/AIDS experience preferred.Â
- Ability to commit to and be flexible in unique care setting involving large number of volunteers.Â

Continuing Education and Training Requirements:Â
- Participates in trainings required by the agency, funding source(s), and/or as required by licensure if applicable
