Community Relations Specialist

The Community Relations Specialist serves as a liaison between the community and City Human Services, with a focus on supporting older adults and promoting access to available services and resources. The position is primarily based at the Sunnyside Health Center and may occasionally visit patients in their homes to check on their well-being and needs.

Schedule:

Monday- Friday, 8:00 am-5:00 pm

Key Responsibilities:

  • Serve as a liaison between the community and City Human Services.
  • Provide information, referrals, and connections to appropriate community resources and services.
  • Support older adults and community members in accessing available programs and services.
  • Provide services and outreach to seniors at the Sunnyside Health Center.
  • Occasionally visit patients in their homes to check on their well-being and needs.
  • Connect seniors with appropriate City Human Services programs and community resources.
  • Facilitate coordination and communication between the senior program and Congregate services.
  • Participate in community outreach and engagement activities.
  • Maintain positive and professional interactions with seniors, patients, families, and community partners.
  • Perform other duties as assigned.

Requirements:

  • High School Diploma or equivalent
  • Basic knowledge of grammar, spelling, punctuation, and fundamental math functions, including percentages and ratios
  • Six months of experience required
  • Valid Texas Driver License required
