Location:Ā  2066 Hudson Avenue, Rochester, NY 14617 (community of the Hudson and North Park Campuses)

Hours Per Week:Ā 40

Schedule:Ā Monday - Friday, 8:30am to 4:30pm with occasional weekends

This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.Ā Ā

SUMMARYĀ 

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.Ā 

Ā 

RESPONSIBILITIESĀ 

•Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment.Ā 

•Assists in the implementation of the patient’s treatment plan under the direction of the RN.Ā 

•Provides routine nursing services for patients as directed by the RN.Ā 

•Complies with the evaluation, treatment, and documentation guidelines of the department.Ā 

•Completes required documentation in an accurate and timely manner.Ā 

•Provides regular patient status updates to the appropriate personnel.Ā 

•Attends and participates in department/facility meetings as required.Ā 

•Acts in compliance with hospital, regulatory, and professional standards and policies.Ā 

•Maintains clinical competency by pursuing continuing education opportunities.Ā 

•Performs other duties as assigned.Ā

REQUIRED LICENSURE / CERTIFICATION SKILLSĀ Ā Ā 

  • Graduate from an accredited School of Practical Nursing.Ā Ā Ā Ā 

  • Current LPN licensure in State of New York.Ā Ā Ā Ā 

  • Basic Life Support (BLS) Certification from the American Heart Association (AHA).Ā Ā Ā

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
