Description
The Integrated Care Team Social Worker provides timely coordination of quality healthcare services to meet individual’s specific healthcare needs in a cost-effective manner; this involves adaptability to meet community members at health centers, in their home or other community sites. Provides and coordinates care with patients and involved people for an appropriate plan that meets the medical, rehabilitative and financial needs of the individual. Works to provide community members support for social determinants of health barriers. Uses motivational interviewing techniques, advocacy skills and knowledge of mental health and community resources to overcome barriers, offers assistance from community organizations, other team members, and care coordination as appropriate. Applies social work methods to provide emotional and mental health support to individuals.Â

Requirements
Â
Education, Training or Certifications Required:
1. Current Maine licensure in social work.
2. Bachelors degree in social work from an accredited institution.
3. AHA Basic Life Support.
4. Valid Drivers License.
Experience Required:
1. Minimum two years of experience in the field of license.
2. Demonstrated knowledge of health, social service, and funding sources.
3. Experience in community and hospital social work and discharge planning preferred.
4. Experience counseling individuals and groups with behavioral, emotional and behavioral disorders.
5. Motivational interviewing experience preferred.
Special Requirements:
1. Within the scope of practice and license, must have the ability to work independently with limited supervision, capability to make independent decisions regarding counseling, education or referral of patients. Maintains confidentiality.
2. Reliable personal vehicle or mode of transportation.

2. Knowledge of best practice standards and ethics.
3. Ability to multitask, prioritize and organize.

1. Able to sit, stand, walk for 8 hour shifts.
