As part of one of New York’s largest nonprofit health and human services organizations, you’ll join a team dedicated to compassion, inclusion, and excellence — helping individuals and families thrive every day. #INDSupport Position Summary: Serves as the primary liaison for patients entering ARS and article 31 clinics. responsible for managing the admissions process, which includes verifying insurance, gathering pertinent histories, and scheduling appointments. They ensure all paperwork is processed accurately while maintaining patient confidentiality Essential Responsibilities: Handle telephone inquiries requesting information about ARS and Article 31 programs; determine the type of service that is being requested and refer to appropriate internal source or appropriate outside agency. Monitor Fax and email for new inquiries and referrals. Maintain a screening schedule which reflects turnaround times in referrals. Obtain and review pertinent data and history on applicants to be considered and admitted to WellLife Network programs. Determine appropriateness of the referral for a specific residence or program. Set up initial screening and program trial visits. Complete data entry in AVATAR. Run Epaces and schedule transportation for first appointments as necessary. Verify insurance for referral. Maintain regular communication with assigned staff at ARS and Article 31 clinics.  Collaborate with Intake Team to process internal referrals coming from PROS or housing. Complete Outreach as directed. Position requires some field work (vehicle required) and In office days. In office days mandatory. Other Responsibilities: Make community contacts with possible referral sources, clinics, in or out patient departments, psychiatrist, day programs, etc. Make presentations at various outside agencies on an as needed basis. Maintain confidentiality at all times. Work locations and schedules are subject to change. Any additional duties as assigned by supervisor.
