Job Purpose: Major Functions of a Care Coordinator are the patient intake process for inpatient treatment, verification of insurance, coordination of transportation for inpatient and outpatient services, obtaining and coordinating referrals.

Essential Duties and Responsibilities:

  • Performs Intake Process for Inpatient Treatment to review existing data and patient information
  • Verifies Insurance Validity and aids in referral process if insurance needs obtained
  • Collaborate with the Transportation Division and coordinates trips for inpatient and outpatient services
  • Obtains and Coordinates Referrals from other Facilities
  • Collaborate with Staff and Providers to coordinate appointments to meet patient needs
  • Resolves work queue issues and manages recall and waitlists.
  • Operates multi-line telephones
  • Collaborate with Office Staff and schedule appointments for On Demand Care
  • Serves all persons served by OVP Health and OVP Health Care
  • Demonstrates competence to provide care for all patient populations: pediatrics, adolescent, adult and geriatrics.
  • Assists in obtaining the patients/clients medical records and files applicable results or correspondence for Provider review as necessary.
  • Maintains safe work environment by following established infection control and safety guidelines. Follows proper equipment cleaning procedures.
  • Attends continuing education and organizational programs assigned and completes annual mandatory education requirements.
  • Other duties may be assigned.

Qualifications/Education/Experience

  • High School Graduation or GED is required, post-secondary education helpful.
  • One year of work experience, preferably in a medical office setting.
  • Medical terminology helpful, customer service skills essential
  • Six months’ customer service experience desired
  • May require BLS for certain locations and/or settings

Work Environment:

The job operates in an OVP Health office setting or an inpatient facility environment.
