Mountain View Hospital is looking for a Social Worker Discharge Planner to join our team!
JOB SUMMARY:
Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care by performing the following duties. Prepare patient and family for discharge from acute care hospital setting.

Qualifications
Education/Certification:Ā Must be a Licensed Social Worker, Bachelor's degree (B.A.) in social service from four-year college or university; Preferred experience in a healthcare facility. BLS Certification required upon hire.
Experience:Ā 3+ years of clinical social work experience.
