Description

The Area Supervisor oversees daily residential services to ensure Person Centered Individualized Support Plans (PCISPs) are implemented effectively while maintaining the health, safety, and well-being of the individuals served. This role ensures compliance with all regulatory and organizational standards, supports and guides staff, and promotes a person-centered environment. The Area Supervisor models and promotes Tangram’s core values while fostering a culture of respect, accountability, and quality service.

  

Leadership Responsibility

  • Advance Tangram’s mission, strategic plan, and values.
  • Provide direction, leadership, performance feedback, and training for assigned staff.
  • Build and facilitate positive relationships with clients, natural supports, teams, professionals, and members of the community. 
  • Participate in ongoing professional development and promote continued development of team members.
  • Identifies opportunities to add new clients, increase occupancy, and expand services to support program growth.


Duties/Responsibilities:

  • Oversee Community Living Managers across multiple locations, ensuring consistent monitoring and implementation of client support plans, review of outcomes, documentation, and regular reporting on client progress.
  • Ensure health and safety of clients including review of medication administration, follow-up on medical appointments, emergency planning, site inspections, and emergency drills. 
  • Ensure staff are well prepared to provide client care and support through direct training, coordinating training, and collaborating with other professionals to ensure training is thorough and effective.
  • Ensure accurate service allocations and utilization of client resources.
  • Obtain and/or complete all admission, enrollment, and training documents for clients.
  • Participate and help guide Person-Centered Individualized Support Plan (PCISP) team meetings.
  • Create meeting notes for client meetings.
  • Create Monthly Reports and upload them to the Bureau of Disabilities (BDS) portal. 
  • Maintain client electronic and in-home records including contacts, financial data, and medical appointments. 
  • Respond to and report critical issues, incidents, and emergent needs.
  • Provide direct support for clients as needed including transportation to appointments, community engagements, and personal care. 
  • Oversee and implement entitlement systems as it relates to Social Security, Medicaid, Medicare, and Section 8 housing, providing needed documentation. 
  • Maintain compliance with service standards and accreditation requirements. 
  • Participate in recruitment, hiring, onboarding, performance evaluations, and ongoing feedback to maintain high performance and team engagement.
  • Review and approve staff schedules to ensure adequate coverage while proactively managing staffing levels to minimize overtime expenses and maintain efficient use of resources.
  • Other duties as assigned. 

Requirements

 Required Skills/Abilities:

  • Strong leadership qualities include excellent communication skills, ability to provide on-call support in a 24-hour setting, demonstrates strong ethical standards, and promotes a culture of learning and growth.
  • Must maintain CPR and first aid certification. Must pass Medication Administration and must stay current on all other training as defined by state, accreditation body, or agency guidelines. 
  • Working knowledge of Microsoft products.

Education and Experience:

  • Bachelor's degree in rehabilitation, psychology, or related major. Consideration may be given to experience and extraordinary performance in lieu of degree.
  • Three years of experience in the human services field with one year of supervisory experience. 
  • Must be 21 years of age or older, must have a valid driver’s license, acceptable driving record, meet agency’s driver insurability requirements, reliable vehicle, and up to date automobile insurance. 

Physical Requirements:

  • This position requires significant bending, lifting, and stooping. 
  • Alternating periods of sitting or standing as dictated by client needs.
  • Must be able to lift/transfer individuals and physically assist clients with movement and care.
  • Ability to lift a minimum of thirty-five pounds.
  • Repetitive physical activity
