The Role Itself
Assumes primary responsibility for assigned client caseload.
Ensures clear, concise, professional, and timely documentation for discharge coordination.
Acts as a liaison between referral sources and clients, conducting family interviews and arranging discharge transportation.
Completes provision of discharge documentation and necessary Release of Information (ROI).
Provides crisis intervention and plans aftercare for assigned clients, including case management.
Assists with Outcome Measures and Satisfaction Surveys.
Education:
Bachelor's degree in Mental Health preferred;Â
High School diploma accepted with experience in mental health population.
Must be 21 yrs or older.
Could hold LSW, LPC, LISW
Could be therapist, counselor, social services.
