At Franciscan, our Hospice Intake Coordinator receives admissions requests for patients requiring hospice care services. This role serves the purpose of coordinating and completing all admission requests.

WHAT YOU CAN EXPECT

  • Schedule visits with field staff to assure that the frequency of visits are compliant with the plan of care.

  • Assist billing in tracking non-paid claims, including researching information and documentation for insurance-related billing problems.

  • Assist patient, families, physicians, and other referral sources via telephone with clinical issues, problems, and questions.

  • Identify and implement clinical review systems for all pay sources to include admissions, continued stay, and concurrent and retrospective reviews.

  • Communicate with all referral sources to gain the necessary information to facilitate continuity of care and appropriate care of patients and clients, including demographics, diagnoses, surgeries, hospital/ECF dates, physicians orders, and reimbursement or insurance information.

QUALIFICATIONS

  • High School Diploma/GED - Required

  • 2 Years Home Healthcare - Preferred
