Position Summary: Reporting to the Sales Manager and/or the Executive Director, you will use your marketing skills to identify, attract, and engage qualified clients for our Structured Family Caregiving Program and Traditional Home care. Building relationships with local hospitals, clinics, case management companies, senior living facilities, etc. As a Health Coach you will have the stability of a base salary plus the unlimited earning potential of commissions. Responsibilities: Engage, motivate, coach, and educate caregivers via phone and digital technology. Assess caregiver and care recipient needs; coach and empower caregivers to develop and achieve goals. Minimum Monthly Check-ins/ or as needed in collaboration with key stakeholders, determine caregiver and client eligibility; problem solve to efficiently move families into service. Face to Face Semi-annual meetings with clients, caregivers, and case managers. Track and manage program referrals/participation to support state and company growth, supplying program specific information as needed. Research opportunities for referral to external support and services. Actively recruit new Primecare Home Care members, create brand presence throughout your assigned area. Communicate with key referral sources, case managers, physician offices, hospitals to spread word about Primecar Home Care services to meet their member’s needs. Document and report on coaching activity with a focus on quality and outcomes; make recommendations as appropriate. Escalate issues when appropriate to the coaching manager as it pertains to consumer changes in status. Proactively identify and cultivate a minimum of five new prospects every week. Foster and nurture relationships with new referral sources to expand the network. Allocate a minimum of four days a week for active marketing efforts. Demonstrate proficiency and confidence in cold calling potential clients. Qualifications: Associate's Degree or 2+ years of experience working community health, long term care, and/or chronic diseases of adults. 2 + years of experience in case management, experience working with elderly and disabled. Exceptional active listening and communication skills with strong empathy Tech savvy with comfort using (and teaching others to use) mobile apps. Ability to multitask in a multi system environment; proficiency with Microsoft Office required. Thrives in a fast paced, start-up environment. Sales experience and building relationships and pipelines. Preferred Qualifications: Prior experience as a Home Health Marketer or Sales related to healthcare preferred. Bachelor’s degree in a related discipline or equivalent work experience in Social Work, Behavioral Health. Prior experience working with consumers in a home base setting. Prior experience in hospice and health care direct marketing is a plus.
