What You'll Do

  • Conduct comprehensive wound assessments, including staging, measuring, and evaluating healing progress

  • Develop and implement individualized wound care plans tailored to each patient's needs

  • Perform advanced wound care techniques such as wound packing, debridement (based on scope), VAC therapy management, and dressing changes

  • Collaborate with physicians, therapists, caregivers, and interdisciplinary team members to ensure continuity and excellence in care

  • Educate patients and families on wound prevention, infection control, and at-home care strategies

  • Document all care thoroughly, accurately, and in compliance with home health standards and regulatory requirements

  • Monitor changes in condition and promptly communicate updates to the clinical team

  • Promote patient independence, comfort, and safety throughout the healing process

What We're Looking For

  • Active RN or LPN license (state-specific)

  • Prior wound care experience strongly preferred

  • Wound care certification (WCC, WOCN, etc.) a plus

  • Reliable transportation
