Job Summary
We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. House is not smokefree.

● Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility.

● Provide light housekeeping, meal preparation, and companionship in accordance with the patient's plan of care.
● Monitor and report changes in the patient's condition, behavior, or environment to the supervising nurse.
● Follow physician orders and the individualized care plan under RN supervision.
● Accurately document care provided and complete Electronic Visit Verification (EVV) for each visit.
● Maintain a safe, clean, and supportive environment while respecting patient privacy and dignity.
Qualifications/Requirement
- Home Health Aide License/CNA License is required
- High School Diploma or GED
- One year prior professional experience
- Driver’s license required
- CPR certification required
- Ability to pass a criminal background check, TB screening, and required health clearances.
- Strong communication skills, dependability, and a genuine commitment to patient well-being.
