Job Title: Referral Coordinator

Job Summary:

Are you a relationship-driven professional who thrives on building connections and improving access to care? Join our team as a Referral Coordinator!

This role serves as the primary point of contact for community referral partners, helping streamline access to services and strengthen collaboration between external partners and internal treatment teams. While this is not a clinical position, it plays a vital role in ensuring individuals referred for services experience a smooth, welcoming, and timely entry into care.

The ideal candidate is highly organized, customer-service oriented, and passionate about improving system navigation for patients and community stakeholders.

Key Responsibilities:

Community Partnership & Relationship Building

  • Serve as the preferred point of contact for community referral partners.

  • Maintain regular contact (phone and in-person) to strengthen partnerships and improve collaboration.

  • Foster positive, professional relationships with social workers, health departments, law enforcement, treatment providers, and other stakeholders.

Referral Coordination & System Navigation

  • Coordinate and manage incoming referrals from community partners.

  • Ensure efficient assessment scheduling and entry into care for referred clients.

  • Monitor referral progression and support timely connection to appropriate services.

  • Work individually with referred clients to personalize engagement and support onboarding into services.

Communication & Education

  • Promote clear, consistent communication between referral sources, internal treatment teams, and agency representatives.

  • Educate community partners about the organization’s service continuum and access processes.

  • Identify and explore new partnership and service collaboration opportunities.

  • Create an environment that promotes positive interactions among all stakeholders.

Education and Experience Qualifications:

  • Bachelor’s degree in social work, mental health, public health, or a related field preferred.

  • Basic computer proficiency (Microsoft Office and electronic systems).

  • Strong organizational skills and attention to detail.

  • Excellent verbal and written communication skills.

  • Customer-service oriented with the ability to balance compassion and assertiveness.

Employment Requirements:

  • Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint screening.

  • Completion of New Hire Orientation at the start of employment.

  • Completion of all required training (including Relias) upon hire and annually thereafter.

  • Valid driver’s license, acceptable driving record, and current auto insurance.

  • Completion of required professional development trainings to meet DMH standards.

  • Thorough knowledge of program policies, procedures, and regulations.

  • Must be at least 21 years of age to operate a company-owned vehicle.

  • Must be at least 23 years of age to operate a 15-passenger van.

  • Must have reliable transportation if transporting clients in a personal vehicle.

Physical Requirements:

ADA Consideration – Sedentary Work

  • Exerting up to 10 pounds of force occasionally and/or negligible force frequently to move objects.

  • Frequent repetitive hand and arm movement for typing and documentation.

  • Primarily sedentary position with occasional walking or standing.
