We are currently seeking a Licensed Intake Scheduler to join our team in our Stuart, Florida office. This position plays an important role in coordinating patient and caregiver schedules, supporting our clinical team, and helping ensure patients receive the care they need in accordance with their physician's orders.

What You'll Do

  • Coordinate the private duty intake process, ensuring client information, service needs, payer information, and required documentation are collected accurately and timely.
  • Work closely with the Private Care team to coordinate the transition from initial inquiry to assessment and start of services.
  • Coordinate schedules to support timely and appropriate patient visits.
  • Monitor completed patient visits and follow up on outstanding documentation as needed.
  • Provide clerical and administrative support to the Nursing Supervisor.
  • Answer incoming telephone calls, assess the nature and urgency of each call, and handle or direct inquiries to the appropriate resource.
  • Communicate with patients, caregivers, clinicians, and internal team members regarding scheduling and patient care coordination.
  • Monitor and process documentation related to patient visits and services provided by VNA of Florida.
  • Identify scheduling or documentation issues and communicate with the appropriate team members to support timely resolution.
  • Maintain accurate and timely information within the electronic medical record and scheduling systems.
  • Protect patient privacy and maintain confidentiality in accordance with HIPAA and agency policies.
  • Perform other duties as assigned.

 

What We're Looking For

  • Previous experience in private duty home care, private care, home care intake, or client coordination preferred.
  • Intake or referral coordination experience is a plus.
  • Understanding of the unique needs of private duty clients and their families is preferred.
  • Healthcare scheduling or staff scheduling experience is a plus.
  • Medical office or healthcare administrative experience is a plus.
  • Strong organizational skills and exceptional attention to detail.
  • Excellent verbal and written communication skills.
  • Ability to manage multiple priorities and meet deadlines in a fast-paced environment.
  • Strong computer skills and the ability to learn electronic medical record and scheduling systems.
  • Ability to work independently while collaborating effectively with clinical and administrative teams.

  • Commitment to maintaining patient confidentiality and HIPAA compliance.
