As the LCSW Case Manager Supervisor, you will provide operational oversight of care management teams, ensure compliance with DHCS and Managed Care Plan (MCP) requirements, and help build an outcomes-driven, whole-person care model that advances coordinated care within California’s evolving healthcare framework.

Job Summary
We are looking for a qualified Medical Social Worker to join our team! You will play a crucial role in evaluating patients and developing individual treatment plans in collaboration with patients’ physicians.

Responsibilities 
  • Lead, supervise, and performance-manage ECM Care Managers, establishing KPIs aligned with CalAIM quality metrics, productivity standards, and outcomes-driven care delivery.
  • Ensure regulatory compliance with DHCS, Medi-Cal, Managed Care Plan (MCP) contracts, HIPAA, and audit readiness through documentation review and quality assurance oversight.
  • Provide clinical leadership on high-acuity cases, overseeing biopsychosocial assessments, risk stratification, crisis intervention, and individualized care plan approval.
  • Build and strengthen cross-functional partnerships with hospitals, health plans, behavioral health providers, housing agencies, and community-based organizations to improve care coordination and reduce ER utilization.
  • Support program development and Behavioral Health integration by optimizing workflows, identifying service gaps, developing scalable processes, and contributing to strategic growth initiatives within California’s value-based care framework.
Qualifications
  • Bachelor’s or Master’s degree in Social Work, Psychology, Public Health, or related field, with a minimum of 5 years of experience in case management, care coordination, and behavioral health, including leadership of multidisciplinary teams serving high-acuity or underserved Medi-Cal populations.
  • Solid working knowledge of Medi-Cal systems, complex care coordination, community resource navigation in Los Angeles County, and regulatory compliance; familiarity with CalAIM ECM policies, MCP workflows, billing, and documentation preferred.
  • Clinical licensure (LCSW) required; experience partnering with Managed Care Plans and supporting program development or service line expansion preferred.
  • Proficiency in EHR and case management software, data tracking, and Microsoft Office applications; solid analytical, organizational, and executive-level communication skills, with the ability to travel locally within Los Angeles County and maintain confidentiality in a fast-paced healthcare environment.
