Sonic Summary

We are seeking a Physical Therapist to provide care to adult and geriatric patients while collaborating with a diverse clinical team.
The responsibilities include preparing documentation, progress notes, and discharge summaries, as well as establishing patient care plans.
Candidates must possess a valid State Physical Therapist License and have experience in Skilled Nursing Facilities (SNF) or acute care.

We are hiring a Physical Therapist to deliver care to our patients and work with a diverse team of clinical professionals.

Responsibilities:

Provide physical therapy to adult and geriatric patients.
Prepare documentation, progress notes, and discharge summaries.
Establish a plan of care in accordance with patients' needs and capabilities.
Travel to nearby centers as needed

Qualifications:

Valid State Physical Therapist License and Credentials
SNF and/or Acute care experience
