The Patient Care Coordinator holds a pivotal role in managing both patient care and administrative tasks. Their primary objective is to ensure that patients receive exemplary support and assistance throughout their healthcare journey.

Responsibilities:

  • Maintain patient accounts by obtaining, recording, and updating patient demographics and insurance information in the EHR system.
  • Schedule appointments and address patient inquiries via various channels.
  • Handle appointment requests and initiate necessary communications.
  • Address patient needs, including prescription refills, prior authorizations, and medical record transfers.
  • Provide support for patients encountering Telehealth or technological challenges.
  • Manage insurance claims and maintain accuracy of patient records in the Electronic Health Record (EHR) system.
  • Monitor online referral platforms and respond to incoming faxes promptly.
  • Process invoices and submit claims efficiently.
  • Intake workers' compensation claimants, ensuring all necessary documentation is collected and processed for their treatment.
  • Coordinate initial assessments for workers' compensation cases, verifying insurance details and ensuring proper documentation for case management.
  • Communicate with workers' compensation insurance carriers to gather relevant information and ensure timely approval for treatment.
  • Maintain accurate financial records by updating patient financial information, recording, and collecting patient charges, and providing receipts for payments.
  • Notify providers of patients' late arrival; reschedule and/or make other accommodations when necessary.
  • Helps patients in distress by responding to emergencies.
  • Encourage patient compliance with preventive care by reaching out to patients and reminding them when they are due for follow ups, re-evaluations, etc.
  • Other duties as assigned


Requirements


  • High school diploma or equivalent.
  • Spanish speaking is a plus!
  • Minimum of 1-2 years of experience in healthcare coordination and patient advocacy, preferably in mental/behavioral health.
  • Experience with intaking workers' compensation claimants is highly preferred.
  • Excellent communication and interpersonal skills to effectively advocate for patients and collaborate with healthcare professionals.
  • Strong organizational and time-management skills to manage multiple patient cases and prioritize competing demands.
  • Punctuality and efficiency paired with empathy and interpersonal skills.
  • Experience with Electronic Health Records (EHR/EMR) systems.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
