We are currently seeking a Home Care Coordinator for our newly created TRAIL program. TRAIL stands for Timber Ridge Assistance in Living. It is a newly licensed Home Care Agency. As a life care community, Timber Ridge at Talus makes aging in place simple. - Certifications: Must have a validFood Handler's license and be a licensed CNA/HCA or NAR with experience.
General Summary:
The Homecare Coordinator plays a crucial role in supporting our Timber Ridge Assistance in Living (TRAIL) program. Â This position is responsible for coordinating care between clients and caregivers, ensuring that each resident's individual needs are met with compassion and efficiency. Â This position serves as a key point of contact, addressing any questions or concerns from caregivers and clients to promote satisfaction and continuity of care. Â Additionally, the Homecare Coordinator may be called upon to fill caregiver shifts as needed, contributing directly to the quality of service we provide.
Essential Job Duties:
- Coordinate scheduling of client visits in our scheduling system.
- Be a focal point of communications with caregivers and clients.
- Fulfil clients' care plans to the best of their ability with staffing available.
- Appropriately schedule caregivers to minimize gaps in the schedule for ease of caregiver error and payroll.
- Triage calls and escalate issues as needed.
- Primary contact in responding to caregiver requests and needs.
- Clerical tasks (filing paperwork, data entry, etc.)
- Track all schedule changes, notify all relevant parties, and update the computer system.
- Requires heavy telephone work, customer interface, and the ability to juggle multiple priorities in a fast-moving field.
- Reports as a caregiver to cover shift, when necessary, when client load is low
Other Duties:
- Provides relief for caregivers during their scheduled breaks.
- Attends in-service training and education sessions as assigned.
- Performs specific work duties and responsibilities as assigned.
