UVA Health Family Medicine is seeking a full-time Advanced Practice Provider (APP) to join our Pantops Primary Care team.  Providing direct evidence-based patient care, maintaining access to care standards, expanding access to care, and helping patients and their families achieve their health goals.   This role is central to the institution's commitment to providing exceptional primary care.

UVA’s Pantops Primary Care Clinic is a combined clinic with Internal Medicine.  This is a busy clinic that offers primary care services to a diverse patient population over the age continuum.  


The full-time APP in this role will work 8 4-hour templated sessions weekly, with an allotment of 8 hours each week for non-patient facing care responsibilities (completing encounter documentation, pre-charting, follow-up communication, in-basket work, etc.). The primary role is to deliver high quality evidenced- based care to a diverse patient population over the age continuum.

Primary Care is a much-needed service, this community continues to grow and expand.  UVA Family Medicine continues to be committed to improving patient access with ongoing professional growth in our primary care clinics.


Clinical:
- Licensed FNP who can provide direct pt care in a primary care clinic.
- Evaluate, diagnosis and manage acute and chronic care needs for a diverse patient population over the age continuum.
- Address resulted patient labs, imaging, or procedure results within 72 hours of receipt.
- Address patient-initiated concerns via My Chart or phone calls within 72 hours of receipt.

Administrative:
- Document patient encounter at time of visit
- Sign encounters within 10 days of visit
- Address all in-basket messages with 72 hours of receipt. 
- Complete annual mandatory learning by due date

Professional:
- Maintain current clinical knowledge and competency in primary care.
- Participate in ongoing education and training relevant to role and patient care responsibilities. 
- Uphold professional standards of practice consistent with APP licensure and UVA Health policy
- Collaborate effectively with physicians, APPs, nursing, and administrative staff across departments
- Attend monthly provider and faculty meetings.

Leadership:
- Provide system-level support and guidance.
- Serve as a clinical resource and point of contact for learners
- Contribute to process improvement initiatives within Crozet Family Medicine
 

MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.

Experience: Department and unit specific.

Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease.

The pay range for this role is $115,830.00 - $171,163.20 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
