In thisĀ high-impact clinical pharmacy role, yourĀ expertiseĀ will strengthen medication access,Ā optimizeĀ therapeutic outcomes, and support continuity of care across multiple specialty service lines.Ā 

Join UVA Specialty Pharmacy as a centralized ambulatory care clinical pharmacist supporting patients throughout their treatment journey. You will combineĀ longitudinal medication management, patient education, clinical assessment, and interdisciplinary collaborationĀ in a hybrid practice model designed for focused clinical work and professional growth.Ā 

The RoleĀ 

As part of an expanding specialty pharmacy program, you will manage a diverse patient population receiving specialty medications across areas such as oncology, rheumatology, digestive health, cardiology, pulmonary arterial hypertension, and other rare or complex conditions. This is not a clinic-embedded or dispensing role. Instead, you will practice as a centralized clinical specialist, connecting with patients telephonically and partneringĀ virtually withĀ providers, nurses, clinic teams, and medication access specialists.Ā 

Your average day will balance patient counseling, clinical review, follow-up assessments, documentation, and collaboration. You will evaluate therapy appropriateness and safety, help patients understand what to expect from treatment, and monitor outcomes over time. The breadth of the practice offersĀ continuous exposure to evolving specialty therapies and disease states, with opportunities to develop deeper specialization as the program grows.Ā 

Key responsibilities include:Ā 

  • Review specialty medication prescriptions forĀ appropriateness, efficacy, accuracy, and safetyĀ 

  • Counsel newly referred patients on medication administration, potential adverse effects, therapy expectations, and disease state managementĀ 

  • Conduct longitudinal follow-up assessments focused onĀ effectiveness, safety, adherence, and clinical outcomesĀ 

  • Document patient progress and clinical interventions in EpicĀ 

  • MaintainĀ documentation aligned with ACHC and URAC accreditation standardsĀ 

  • Communicate clinical findings and recommendations to providers, nurses, and clinic teams through virtual channelsĀ 

  • Partner with medication access specialists to help address insurance, prior authorization, reimbursement, and affordability barriersĀ 

  • Contribute to quality improvement, medication safety, medication use evaluation, REMS, and performance improvement initiativesĀ 

  • Support the implementation of new specialty therapies andĀ participateĀ in clinical research opportunitiesĀ 

  • Build trusted virtual relationships while independently managing a longitudinal patient populationĀ 

The hybrid schedule includesĀ four remote days and one onsite day each week at Discovery Drive. Patient interactions are telephonic, and provider and clinic collaboration occurs electronically or virtually.Ā

MINIMUM REQUIREMENTS:

Education: Bachelor's in Pharmacy or Pharm.D. from an ACPE accredited College of Pharmacy required.

PHYSICAL DEMANDS:

Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.

The pay range for this role is $64.37 - $89.07 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
